Looking at the play in the removed bearing and comparing it to a new one, it’s definitely loose.
Once assembly was done, let’s see how it sounds…

The chain and sprocket were replaced not long ago, so that should be fine.
I wonder what it could be…
Looking closely, the lock washer isn’t sitting flush against the nut.
That means it’s loose and there’s play between the spline and the washer. I straightened the washer.
When I touched the nut… oh, “it’s loose!”
I tightened it and bent the washers at two spots, and we’re done.
The rear spins smoothly now, nice and quiet!
